Employee Exit Interview

Bluefield University values the input of our departing employees. Therefore, we are asking you to complete the Exit Interview form below. The intent of this Exit Interview is to ensure that our employees are informed of important information regarding their departure, benefits, and final pay.

This form is also used to review administrative details with the employee such as benefit continuation rights and conversion privileges, if any, final pay, and as a reminder to return the following:

  • University Photo ID Card
  • Office keys or any vehicle keys
  • Key to Campus mailbox located in the Campus Store - if any
  • University issued credit cards
  • Computer/phone - any equipment owned by the University
  • Any additional University-owned or issued property.

Please keep in mind that all property including all electronic documents, files, records, equipment, manuals, media, supplies, or similar materials and information stored on the College computers remain property of the University and must be returned intact when leaving employment.

Please complete this form as thoroughly as possible and be advised that any pertinent information disclosed during the exit interview may be shared and/or discussed with the University President or university leadership as deemed appropriate.
