All currently enrolled Bluefield College students are required by the College and/or Virginia policy to provide health information to the Office of Student Development prior to their first semester of enrollment. Failure to submit a completed Pre-Entrance Health Packet will result in delay of moving onto campus and/or attending classes. Students planning to participate in intercollegiate athletics at Bluefield College are asked to complete the Pre-Entrance Health Packet for Student-Athletes. All other students must complete the Standard Pre-Entrance Health Packet.
The Pre-Entrance Health Packets can be downloaded and printed (PDF format) below. Health packets must be returned to the contact listed on the form. Student athlete health packets submitted for the 2018–2019 school year must be completed after June, 1st, 2018.
STANDARD PRE-ENTRANCE HEALTH PACKETPre-Entrance Health Packet for Student Athletes
Completed Pre-Entrance Health Packets must contain the following items:
The Pre-Entrance Health Packet provides a student’s health and immunization history. All immunizations must be up-to-date and immunization documentation must include the signature of a license health professional and appropriate dates. The Health Record also includes full insurance information (company and policy holder information). This will not be accepted as a waiver for the student health insurance.
This also includes a Medical Consent for all students. These forms outline necessary medical information to be released in the event of a medical emergency.
Bluefield College is committed to ensuring students have access to affordable, quality, and comprehensive health insurance. Bluefield College has been partnered with Gallagher Students Health and Special Risk for more than four years, working with students and parents, providing valuable information about student insurance.
ALL TRADITIONAL STUDENTS- INCOMING FRESHMEN, RETURNING AND TRANSFER STUDENTS on campus who are enrolled in 6 or more credit hours per semester and in a degree-seeking program are eligible for the plan. Bluefield College practices a “hard waiver” process. Each academic year the Student Health Insurance (insurance premium) is added to all students’ accounts. If the student/parent does not go online to complete a waiver form by the published deadline of September 1st the student will be enrolled and responsible to pay for the premium charge on their account. If the waiver is completed by the deadline, the charge will be removed. A completed, verified, and approved WAIVER is required each academic year. Open enrollment/waive period will begin in June through September 1st.
To learn more about our student health insurance (eligibility, benefits, cost, and waiver form) please visit the Gallagher website at www.gallagherstudent.com/bluefield. We strongly encourage students to contact our office with questions regarding the waiving and enrollment process. Our goal is to see that all students who wish to be enrolled in the insurance are enrolled in a timely manner and for those students who have their own insurance coverage and are able to waive the insurance, we strive to have the charged removed from their accounts as quickly as possible. When you receive your statement from the Bluefield College Business Office, the insurance adjustment will be made to your statement.
Should you have any questions, please contact the Office of Student Development at 276.326.4207.
When flu season is underway, it is important to be informed and aware of preventative measures, symptoms, and treatment options. Always cover your coughs and sneezes, and keep your hands washed to help prevent the spread of germs. Be reminded that typical flu-like symptoms include headache, chills, body aches, fatigue, dry cough, sore throat, stuffy nose, and a fever. In addition, if you do become sick with the flu, stay home to rest and prevent spreading the illness. If you remain fever-free while being off any fever-reducing medications for a 24-hour period, you may resume normal activity without fear of spreading the flu.
For more information, visit the websites for Centers for Disease Control and the Virginia Department of Health.
Flu Clinics are held each semester and flu vaccines containing the H1N1 vaccine are provided for students, faculty/staff, and their families.
