These forms are required to be signed and dated before participation in athletics at Bluefield College. If the student-athlete is under the age of 18, a parent or legal guardian must also sign the form in the designated areas.

Please take your time to fill out all the forms in their entirety.

Please check with your coach for the date, time and location of your team's pre-season physical.

Please have the above forms filled out prior to coming to physicals.

No athlete will be allowed to practice unless ALL forms have been submitted to the Athletic Training Department.
